Thyristor module by Eupec 1200V 18Amps. The case is the powerblock and is overall 3.62" long x 0.8" wide x 1.2" tall. Diagram of connections is on side of module. The connections are via the 3 threaded holes on top and 2 numbered eyelets on top. New - Retails for up to $40.
